Traffic-light system

Communication

Coordination is part of care. Instead of scattered phone calls and private messenger groups, everyone involved writes directly in the app – one to one or in a group for each patient.

Who can reach whom

  • Family members and the patient can reach the care staff and administration of their home care service
  • Care workers can reach colleagues in their own organisation and the families of their patients
  • Anyone who is not involved with the patient cannot be reached – not even with the ID
  • When a family member’s assignment ends, the connection ends too
  • Messages are stored on our server in Frankfurt am Main, not on private phones
Mobile care documentation

Mobile care documentation at the bedside

The care worker records what was done while with the client – on the phone, with no catching up on the PC later. Everything feeds into the care report, the medication overview and the reports for the long-term care insurance fund (Pflegekasse).

Available in the app today

  • Daily medication plan per patient with times and a counter (e.g. 1/2)
  • Sign off a dose with one tap – time, dose and care worker are saved
  • “Not given” only with a reason (refused, asleep, out of stock …)
  • Prompt if the same medication is signed off twice within 10 minutes
  • Care report with type, urgency and a note for the next shift
  • Dictation for care reports and wound notes: speech recognition runs on our server in Frankfurt – the recording is not sent to any cloud service and is deleted after recognition
  • Offline records: the records of the patients on the route are loaded onto the phone in advance; without a connection, the app shows the saved version with its timestamp
  • Documentation continues without a connection – medication, reports, proof of service with signature, wounds with photos, arrival and departure. Everything is sent to the server automatically with the time it was recorded as soon as there is reception again
  • Today’s care plan: tick off tasks or mark them “not done” with a reason
  • Shift handover across all your own patients: urgent reports, “next shift please check” items to acknowledge, medication not given, overdue tasks, wounds with signs of infection, new risks
  • Risk assessment: pressure ulcers using the Braden scale (score and level), fall risk as a list of factors based on the national expert standard on fall prevention – where there is a risk, measures are mandatory, and older assessments are flagged for reassessment
  • When logging out, patient data is deleted from the phone; entries not yet sent remain secured and are only sent under the account of the person who recorded them
  • History of reports from the last 14 days with author and shift
  • Arrival and departure per visit in “My shift” – without GPS tracking of staff
  • Proof of service (Leistungsnachweis) per visit: tick off tasks, add extra services from the federal state’s service catalogue
  • Signature by the client or a representative with a finger; without a signature only with a reason
  • Confirmation as a record: cannot be changed, corrections only by cancellation by the nursing manager, with a checksum
  • The signed confirmations appear in the proof of service for the long-term care insurance fund (PDF)
  • Wound documentation: body site, wound type, pressure ulcer category (EPUAP), cause
  • Wound progress with photo, length × width × depth and area, wound bed, exudate, wound edge, signs of infection, pain, dressing and next dressing change
  • Trend at a glance: is the wound getting smaller or larger? Due dressing changes are flagged
  • Wound photos are stored only on the server in Frankfurt, not in the gallery of the work phone; metadata such as the location where the photo was taken is removed

Who may do what

  • Care workers, care assistants, management and doctors may document – family members and patients may not
  • Only patients the care worker is responsible for
  • Only medication from the active plan can be signed off
  • The author is always the logged-in account – this cannot be changed
  • Arrival and departure only for visits on your own route

In development – not yet in the app

  • Transmission to the long-term care insurance funds via the telematics infrastructure (KIM). From 1 October 2027, home care services will bill services under SGB XI electronically; until then, paper records are still permitted. The signed proof of service on the phone is already available.
