How points become a care level

The assessment rates 64 criteria in six modules. Each criterion receives 0 to 3 points – from “independent” to “dependent”. The total per module is not simply added up but converted into weighted points using a table. Only their sum determines the care level (Pflegegrad).

That is why relatives often cannot reconcile their own estimate with the decision: five extra points for mobility can have no effect at all, while five extra points for self-care can mean a whole care level.

If points are missing

If the calculator shows that only a few points are missing for the next level, documentation is the lever – not argument. A care diary kept over two to four weeks proves what gets lost during the appointment, especially in module 3 (restlessness at night) and module 4 (self-care).

If you already have the decision and it differs significantly from your calculation, the way forward is an appeal or a request for a higher care level.

Monthly amounts, as of 2025. Care allowance and in-kind care benefits can be combined; the relief allowance is earmarked and is reimbursed, not paid out.

Frequently asked questions about the care level calculator

How accurate is the calculator?

It uses the thresholds from § 15 SGB XI and the weighting tables of the assessment guidelines – the same calculation the Medical Service uses. The uncertainty lies not in the calculation but in how you estimate the individual points.

Where do I get the individual points from?

From the assessment form. Each criterion is rated with 0 to 3 points (independent, mostly independent, mostly dependent, dependent). Enter the total for each module here.

Why does module 3 sometimes not count?

Modules 2 and 3 share a weighting of 15 percent. Only the higher of the two values counts. Otherwise the maximum would be 115 instead of 100 points.

Are my entries saved?

No. The calculation runs entirely in your browser; nothing is sent to us and nothing is stored.

Is the result binding?

No. It is a guide. The binding document is the decision issued by your long-term care insurance fund after the assessment.
